Damaged/Missing Items Report

We do our best to package your order carefully, but sometimes it can become damaged or be forced open along the journey. Thank you for completing this questionnaire, which helps us recoup our loss from the carrier to keep our shipping costs low for everyone.

In order to file a claim, our carriers require us to collect the following information. PLEASE DO NOT DISCARD YOUR PACKAGING, LABEL, OR DAMAGED ITEMS for a few days in case they ask for additional information.
